Social Services/Discharge Planning
Social Services/Discharge Planning is a service offered to all patients admitted to Providence Medical Center.
This service includes:
 arranging home medical equipment
arranging home health/personal care support
assisting with nursing home placement or alternate living arrangement prior to discharge
assessing the patient’s financial resources and assist with referrals to all community services available based on the patient’s needs
assisting with applications for financial assistance
Our staff works closely with the physician, patient, and family to ensure that the individuals needs are met prior to discharge.
