| Social Services/Discharge Planning is a service offered to all patients admitted to Providence Medical Center. This service includes:  arranging home medical equipmentarranging home health/personal care supportassisting with nursing home placement or alternate living arrangement prior to dischargeassessing the patient’s financial resources and assist with referrals to all communityservices available based on the patient’s needs assisting with applications for financial assistanceOur staff works closely with the physician, patient, and family to ensure that the individuals needs are met prior to discharge.
